Prescribing Physicians
Please note that the prescription you write for your patient must meet the following requirements:
- Patient’s full name
- The date of the RX
- Name of product needed must be listed
- Corresponding diagnosis that qualifies patient for their chair must also be Listed.
- Length of Need must be 99 years
- The date of the "Face to Face" with patient must be clearly documented in the Patient’s file. There must be notes saying the subject was discussed with the Patient. This date must be written on the RX.
- Signature on the RX can be electronic but not a stamp.
After the prescription is written and delivered to Whitley Home Medical Staff, the Prescribing Physician will be sent:
- Letter of medical necessity to be signed by prescribing physician and returned to Whitley Home Medical.
- Equipment list (included in Letter of Medical Necessity) to be signed and returned to Whitley Home Medical.
* Written notes regarding the patient's need that correspond with the date listed on the prescription of the "Face to Face" must also be remitted to Whitley Home Medical with the above documents.
Physical Therapists
We understand that the recommended equipment for a patient's condition may fall outside of their insurance coverage. In the effort to provide the best, and most achievable care for our patients, Whitley Home Medical staff will be available to advise on specific equipment / services that each patient's insurance will provide as well as inform on which diagnoses qualify for which chairs.
